ML5 Seafarer Medical Report

 

ML5 Medical Report - £100

  • What is the ML5 medfical report form and certificate?

    • The ML5 Medical Report is required by the Maritime and Coastguard Agency (MCA) for certain seafarers

    • The purpose of the ML5 form is to obtain a factual report of your medical history and present state of health, enabling the doctor to decide on your fitness to navigate safely and undertake emergency duties

    • If you are fit and safe to work at sea an ML5 certificate will be issued at the conclusion of the medical​​ appointment

    • If the doctor is not able to issue an ML5 certificate at the conclusion of the appointment they will provide you with the completed ML5 report and you can request to have your case reviewed by an MCA Medical Assessor

  • Who needs an ML5 Medical Fitness Certificate?

    • the master of a passenger ship that does not go to sea (Class IV or V) ​

    • the master of a commercial vessel that does not go to sea 

    • the master or a crew member of a small commercial vessel certificated for Area Category 2 to 6 (no more than 60 miles from a safe haven)

    • a crew member or anyone else (for example catering staff) who normally works on a domestic passenger ship (Class VI or VIA) that goes to sea

    • Master or Crew of fishing vessels under 24m, operating not more than 200 miles from the coastline of the UK or beyond the Continental shelf, and that are not at sea for more than 72 hours (required from 30th November 2023)

  • What do I need to bring with me to the ML5 Medical Appointment?

    • It is essential that you bring photographic ID to the appointment​

    • Please also bring along the completed medical history questionnaire which you will receive by email when you book an appointment

    • If you use glasses or contact lenses please bring these with you for the Vision Assessment

    • If possible please ask your GP Practice for a summary printout of your medical history and bring this with you to the appointment

    • If you take any medications please bring a list or original packaging with you to show the Doctor
